Idaho Inspection Reports

These reports come from the Idaho Department of Health and Welfare, Division of Licensing and Certification, which licenses children’s residential care facilities and outdoor programs and posts every survey in its public documents library: a statement of deficiencies with each rule broken, what the surveyor found and the facility’s plan of correction, or a letter saying no deficiencies were found. The state does not say which surveys followed a complaint. Facilities that have left the state’s provider list keep their reports here.
