These reports come from the Maine Department of Health and Human Services, Division of Licensing and Certification, which licenses behavioral health organizations and lists every survey in the state’s licence lookup: full agency surveys, desk reviews and complaint surveys with their outcome since about 2013, and since late 2024 the documents themselves, with each deficiency and the organization’s plan of correction. The licence covers a whole organization, not one site, so a survey can concern any of its programs; surveys about adult programs are hidden unless you choose to see them.

Only organizations that run programs for young people are listed here. Maine’s separate licence for children’s residential care facilities has nothing online, and a survey listed as having an accepted plan of correction found deficiencies even where its statement is not online.
