  Based on record review and staff interview, the facility failed to provide documentation of a satisfactory preliminary records check determination prior to a person serving as an employee in one of five staff records reviewed. Findings Include: Review of Staff E's file on June 17, 2026 at approximately 1:20 p.m. revealed that the criminal records check, dated February 4, 2026, needed further evaluation. There was no additional documentation in Staff E's file verifying a satisfactory final determination was received. Staff E was hired on February 12, During an exit interview with Staff F on June 17, 2026 at 3:30 p.m., Staff F acknowledged the findings.
