      In a review of staff files, it was noted that there are two CHU violations. A staff member was rehired on 8/6/21 and an ISP was not completed until 12/1/21 which exceeds the 30-day requirement. A staff was cleared on 10/12/21 and the clearance letter was not printed until 11/3/21 which exceeds the 14-day requirement. This is a repeat deficiency.

      The child received a head injury on 7/25/22. Two Cornerstone staff witnessed the head injury which was caused when the child was assaulted by another resident and her head was hit to the ground and punched. A Cornerstone administrator witnessed the child being assaulted through video monitoring on his cell phone. The child was seen by the Cornerstone nurse on 7/26/22 at 2pm. The child continuously complained of a headache and was nauseated. She was seen by DHW contract monitor Patti W. on 7/27/22 and she requested the child be taken to the doctor. The child was seen by the doctor on 7/27/22 and was diagnosed with a concussion and prescribed brain rest. Brain rest was described as:no screen time, no reading and rest in a dark room for 72 hours. IDAPA 16.06.02.544states: An organization must determine, organize and deploy the human resources needed to provide services subject to applicable rules and to promote optimum outcomes for persons served. An organization must have an adequate number of qualified administrative, supervisory, social service, direct care staff and other staff to perform the prescribed functions required by applicable rules to provide for the needs, safety, protection and supervision of children served. Cornerstone staff, who witnessed the child receiving ahead injury, did not provide for the needs of the injured child as evidenced by staff not seeking immediate medical care for a head injury.
