      Program staff self-reported that a resident had self-inflicted wounds and had been taken to the hospital. The licensing coordinator's review of the Incident Report and an email sent to all staff on Saturday, July 25, 2026, regarding the safety plan for the resident revealed that the resident was discharged back to the program. The program developed a safety plan for the resident, including five- to 10-minute bed checks to ensure the resident's safety, and that the resident was not to receive any additional items in their bedroom, as the program needed to ensure that no items would be potentially harmful. Additionally, Staff A stated that the safety plan had been discussed with the staff responsible for the resident in person upon the resident's return from the hospital. Staff B stated that while reviewing video footage, it was observed that the staff member assigned to the resident brought the resident a cell phone and charger, and failed to complete the five- to 10-minute checks outlined in the safety plan. The program terminated the staff member responsible for the resident on the night of the incident and will be reviewing the importance of safety and treatment plans with all staff.
