  The facility was found in non-compliance based on interviews with staff and youth; it was confirmed staff hugged and made inappropriate comments to the youth. A staff member also witnessed another staff hugging a youth and failed to report this to their immediate supervisor.

  The agency is found to be in non-compliance as through interviews, video and documentation reviewed, it was found Staff 2 did not perform a proper restraint hold on Youth A. Staff 2 had his arm around Youth A’s neck in a chokehold position and pushed Youth A’s face with his hand. These are not appropriate CPI restraint holds.

  The agency is found to be in non-compliance of dispensing medication. Youth A’s medications are prescribed to be taken during bedtime, however his medication was dispensed to him several hours before his bedtime.
