      Staff 1 and Staff 3 are required by rule to document each observation he had with Youth A. Staff 1 only documented time intervals for every 15 minutes. These times were not the actual times he observed or interacted with Youth A. Along with this, Staff 1 did not document all the interactions. Staff 3 did not document the 2:12pm and 2:39pm observation as the noted times. The staff appear to be documenting that 15-minute checks are completed even if they do them earlier than 15 minutes or later. Consultation: Supervisor 1 and Administrator 1 both reported staff cannot open a door to seclusion unless there is a second staff. This is not documented in their policy. If the agency wants to 12 maintain this as their procedure, they will want to clarify this in their policy.
