A father announced plans to file a wrongful death lawsuit against the McGraw Residential Center in Seattle following his 15-year-old daughter's death by suicide in January 2008. State investigators and the Department of Health concluded that staff members failed to conduct mandated five-minute safety checks and subsequently suspended their credentials.
