Agency staff admitted that medication for the overnight visit was not packaged and labeled as required by agency policy.

Violation established, special investigation : CCI Rule 400.4160 Emergency restraint
The agency is in non-compliance with this rule. • Staff 1 used a CPI transport position to intervene with Youth A. Youth A was seated when Staff 1 used this intervention and was not a threat to self or others. This position is used when youth are regaining control and after a team control position. • Staff 2 swung her arm at Youth A, although not making contact, this is not supportive stance or protective stance. CONSULTATION: The agency “Crisis Plan” and Crisis Prevention Institute Policy are specific and well written. It may benefit the agency to review these as a training to their staff either in individual supervision or in team meetings regularly. All staff are to know the details of the Crisis Plan and be able to walk through these interventions during their shifts. It may also help the agency recognize those individuals who need more training in this area.

At the conclusion of the investigation, the agency was found to be in non-compliance with the licensing rule. Interviews were completed with both youth and staff at the agency. Staff 1 reported her intent to "protect herself" and admitted threatening the youth and stated if he hit her, she was going to hit him back. Technical Assistance: • Recommendation for agency to train staff to include witnesses in the incident report and have other staff present to also complete an incident report for detailed documentation.
