Based on record review and staff interview, the agency failed to provide documentation that the casework supervisor possessed the required educational qualifications in one of three personnel records reviewed. Findings include: A review on August 6, 2025 at approximately 11:20 a.m. of Staff C's record, revealed documentation of a bachelor's degree in science. Further review revealed that there was no documentation that a waiver was not obtained for Staff C. Staff C was hired as a Family Consultant on April 5, 2018 and promoted to Casework Supervisor on November 18, 2024. During an exit conference with Staff A, Staff B, and Staff on August 6, 2025 at approximately 3:25 p.m., after review of the records, Staff A, Staff B, and Staff C acknowledged the findings. Staff A indicated that he/she was not aware that a casework supervisor must possess a master's or bachelor's degree in the area of social work, psychology, childhood education, special education, guidance counseling, behavioral or social science, or related field.
