      The facility is found in noncompliance as Youth A was placed in an unwarranted restraint due to trying to throw a rock to break Supervisor 1’s car window. Technical Assistance: An escort is only to be used to guide a youth, while the youth has free movement. If the youth does not have free movement due to staff holding youth or forcing them to go somewhere the youth lacks free movement and this is considered a restraint, which requires the restraint procedure to be followed.

      The facility is found in non-compliance of maintaining ratio. Based on interviews and video, the facility does not maintain the ratio of one staff to supervise three youth. Rather the facility was staffing 1 staff for three youth in the building and not in direct supervision as interviews and video indicate more than three youth were allowed to be upstairs with one staff.

      The facility is found in non-compliance as there is not sufficient staff to address the issues occurring in the home. Currently the facility is staffing the house based on the ratio of one staff to three youth, however due to the current increase in incidents in the house this has caused staff to need to address incidents resulting in staff not being able to supervise the other youth in the home. TECHNICAL ASSISTANCE: The facility should address the concern of peers providing first aid to peers as staff who are trained in first aid should be providing first aid to youth.

      The facility is found in noncompliance as per youth and staff interviews, Youth C’s safety plan review, as well as video review, staff did not supervise Youth C the entire time she was in the backyard. Staff 3 followed Youth C outside but failed to keep Youth C in line of sight as Staff 3 came back into the house leaving Youth C unattended for approximately six minutes. Earlier in the day Youth C had been to the hospital for saying she wanted to kill herself and stated she overdosed on medication. TECHNICAL ASSISTANCE: Technical assistance was offered to train staff when youth are actively self-harming in front of staff and how staff are to best assess the situation and respond.
