Out of compliance, complaint, investigation inspection : R501-1-8(1)(a)-(i) Facility and safety requirements

The provider was out of compliance with R501-1-8(1)(a) for failing to maintain clean mattresses and ensure the environment is free from health hazards. During the inspection, the evidence substantiated mattresses with visible urine stains and what appeared to be vomit residue. Maintaining stained or contaminated bedding compromises facility sanitation and exposes clients to potential biohazards and unhealthy sleeping conditions. This is a repeat rule noncompliance as noted on February 25, 2024, December 5, 2024, April 10, 2025, and July 28, 2025.Technical Assistance was given on November 25, 2025 and July 28, 2025.

Out of compliance, investigation inspection : R380-80-5(10) Services and supervision to meet each clients needs

The provider was out of compliance with R380-80-5(1) by not providing services and supervision commensurate with a client’s behaviors and needs. During the investigation inspection, the evidence substantiated that a client with a documented history of self-injurious behavior engaged in two self-harm incidents. On August 7, 2025, the client's severe self-harm required transport to the emergency room for treatment and necessitated multiple sutures. Three days later, the client was not adequately supervised and self-harmed again by removing multiple sutures from the prior injury. This incident resulted in a second transport to the emergency room for additional treatment. This noncompliance was previously noted on 07/23/2023, 01/24/2024, 07/15/2024, 10/22/2024, and 12/16/2024.

The provider was out of compliance with R380-80-5(1) by not providing supervision commensurate with clients’ behaviors, and needs. During the investigation inspection, the evidence substantiated that on September 17, 2025, a client with a history of significant self-injurious behavior engaged in a serious self-harm incident that resulted in a transport to the emergency room which necessitated over 30 sutures. A second client engaged in self-harm with the same contraband which also resulted in transport to the emergency room and sutures. This noncompliance was previously noted on 07/23/2023, 01/24/2024, 07/15/2024, 10/22/2024, and 12/16/2024.

The provider was out of compliance with R380-80-5(1) by not providing supervision commensurate with a client’s skills, abilities, behaviors, and needs. During the investigation inspection, the evidence substantiated that a client with a history of significant self-injury, including 3 recent incidents, obtained contraband and engaged in a serious self-harm incident on October 30, 2025. The client's injuries resulted in a transport to the emergency room for treatment which necessitated multiple sutures. This noncompliance was previously noted on 07/23/2023, 01/24/2024, 07/15/2024, 10/22/2024, and 12/16/2024.
