      The agency is found in non-compliance as per videos reviewed, Staff 1 did not complete bed checks as required. Technical Assistance Technical assistance was offered as follows: - The facility should not allow staff to work alone if there are concerns with a staff’s performance leading to potential safety issues. - Facility’s bed check policy should be updated to reflect eyes-on checks must require a staff to see a part of a youth’s body to identify a youth is in their bed and should not include “body size and clothing worn at bedtime”. Consultation: Consultation was offered to the facility regarding the following: - If staff sit with their back facing the mural wall adjacent to the bedroom hallways, there is limited to no visibility into the hallways. - Keeping the kitchen door leading to the bedroom hallway closed with a chime or bell on it may deter youth from entering the opposite gender’s hallway from the kitchen, which is out of staff’s sight if staff are in the recreation room. - Staff’s use of noise cancelling headphones, air pods, etc. while they’re on shift may prevent them from being able to hear or be alerted to youth activity, safety concerns, etc. 7 - Since the beds are high enough for youth to fit underneath, the facility should consider updating the bed check policy to include checking under beds to ensure the room is occupied by only those youth who should be there if youth aren’t accounted for elsewhere.
