The program self reported that the health advocate responsible for medication for the residents, did not comply with the policies of the program in regards to resident medication. The licensing coordinator's review of the program's Internal Incident Review form and the Critical Incident Report revealed that this resident had previously been prescribed 150mg of Trazadone, where 1/2 tablet equaling 75mg was to be taken nightly, and the health advocate was giving the resident 150mg nightly. The Health Advocate stated they had misread the prescriber's information and decided to continue giving 150mg nightly until the next medication check. The program verified that this decision was not made per policy which is to obtain DCYF approval. The program sent the resident to the Emergency Room to have the medications adjusted and have them evaluated for the increased dosage, terminated the health advocate and has also started an internal medication audit to confirm all the prescriptions for residents are accurate.

Founded, problem resolved, complaint visit : He-C 4001.15(w) No person other than a licensed practitioner shall make changes to the written order of a licensed practitioner regarding prescribed medication.
The program self reported that the health advocate responsible for medication for the residents had misread the prescriber's information when requesting a refill. The prescription was 150mg tablet and 1/2 of that tablet equaling 75mg to be given nightly and the refill requested was 150mg nightly. Upon realizing the mistake, the health advocate continued to give the increased dosage until the next medication check. The program confirmed that the health advocate is not a licensed practitioner, therefore not allowed to make these changes to medication themselves. The program sent the resident to the Emergency Room to have the medications adjusted and have them evaluated for the increased dosage, terminated the health advocate and has also started an internal medication audit to confirm all the prescriptions for residents are accurate.

The program self-reported that a staff member left two residents unsupervised during an off-site field trip on October 4, 2025. The Licensing Coordinator's review of documentation provided by the program, including emails written by the staff and notes of conversations with staff and residents, revealed that the staff member admitted to allowing one male and one female resident out of their sight and sound during an off-site field trip. Additionally, the staff member admitted that they violated program policies regarding co-ed off-site trips and the 1:4 staff-to-child ratio requirement when they took three male and two female residents off site. The program terminated the staff member on October 9, 2025, after the staff member reported that they did not supervise two residents, failed to complete a critical incident report, and for non-compliance with program policies regarding co-ed field trips and staffing requirements.

The complainant reported that a resident returned to the program without their prescription medication after a home visit over the weekend. Additionally, when program staff prepared the resident's medication prior to leaving for the visit, they provided the entire prescription and not just the medication necessary for the time that the resident was not going be at the program. The licensing coordinator's review of program emails composed on May 30, 2024 revealed that upon return to the program from a home visit, staff did not verify that a resident's unused prescription medication was returned.

The complainant reported that a resident returned to the program without their prescription medication after being on a home visit over the weekend. Additionally, when program staff prepared the resident's medication prior to leaving for the visit, they provided the entire prescription and not just the medication necessary for the time that the resident was not going be at the program. The Child Care Licensing (CCLU) and Health Facilities Administration (HFA) licensing coordinators reviewed program medication records and several emails regarding this allegation which confirmed that a resident returned from a home visit without their prescription medication, and that the resident went for approximately 15 days without having the medication.
