Non-compliant, complaint visit : He-C 4001.22(b)(11) Program staff shall not use rough handling on residents, including but not limited to, grabbing, pushing, pulling, and dragging.
Program staff self-reported an incident that occurred in the program on March 3, 2026. Program leadership was notified by school staff that Resident A reported that a staff member grabbed them by Resident A's hands and struggled with Resident A, which ultimately caused Resident A to fall to the ground. The licensing coordinators' review of video footage provided by the program revealed that, in response to Resident A becoming upset and throwing their boots into an empty area of the common room, Staff A walked up to Resident A and grabbed Resident A by their hands, and did not release Resident A when they attempted to pull away. The video showed that this struggle lasted for approximately 13 seconds, until Staff A released the child's hands, causing Resident A to fall backward onto their buttocks on the ground. Staff A stated that they took hold of Resident A's wrists to help Resident A calm down. Statements by Staff B and Staff C indicated that the physical interaction between Staff A and Resident A was not consistent with any approved restraint technique. Program administration provided documentation indicating that their review of the incident determined that the technique used was neither approved nor taught by the program, and that it had a high potential to cause injury to the child.
