Based on record review and interview, the administrator failed to ensure a resident's medical record contained orders for one of three resident records sampled. The deficient practice posed a risk as assistance in the self-administration of medication could not be verified against a medication order. 1. A review of R2’s medical record revealed a daily medication administration record (MAR). The MAR revealed R2 was provided "Cetirizine HCL 10 mg tab" on July 2, 2026 at 7:14 am and July 3, 2026 at 7:49 am. 2. A review of R2's medical record revealed no medication order "Cetirizine HCL 10 mg" 3. In an interview, E1 reported the medication was an over the counter medication, and the facility would obtain a medication order for the medication. 4. In an exit interview, the findings were reviewed with E1, and no additional statements or documents were provided.

Based on observation and interview, the administrator failed to ensure a shared resident bedroom contained at least 60 square feet of floor space, not including a closet, for each individual occupying the shared bedroom. 1. The Compliance Officer observed two beds in a bedroom located on the second floor of the facility, directly across from the stairwell. Using an electronic measuring device, the Compliance Officer measured the bedroom to be 119 square feet. 2. In an interview, E6 acknowledged the bedroom did not contain at least 60 square feet of floor space for each individual occupying the bedroom.
