Hawaii Inspection Reports
← View the full Hawaii state page (programs, news, lawsuits, legislation)
These reports come from the Hawaii Department of Health, Office of Health Care Assurance, which licenses special treatment facilities and therapeutic living programs and posts each licensing inspection since 2023 on its inspection reports page: a statement of deficiencies with each rule broken, what the inspector found and the facility’s plan of correction, or a statement that no deficiencies were found. The same licences cover adult programs; only programs for young people are listed here.
Most statements with deficiencies are scans of the signed form. Their text was read by software and may contain reading errors, and plans of correction written by hand are not transcribed; the state’s document is linked with every report. The state does not say which inspections followed a complaint, and posts no complaint investigations.
