      An unannounced Psychotropic Medication Inspection was conducted by Licensing Program Analysts (LPAs) Reyna Martinez and Melissa Renovato on May 20, 2026 at 12:35pm. This case management inspection is conducted pursuant to Health and Safety Code 1538.9(a)(2). During the inspection, LPAs met with Sabrina Sanchez, Direct Care Staff. The purpose of the inspection was to conduct client interviews as clients were not available for interview during the initial inspection on May 7, 2026. The clients declined to be interviewed during this inspection. LPAs provided a copy of the licensing report to Direct Care Staff, Sabrina Sanchez. No deficiencies are being cited at this time. LPAs conducted exit interview with Sabrina Sanchez, Direct Care Staff.

      On April 15, Licensing Program Analysts (LPAs) Miriam Flores and Olivia Medrano met with Facility Manager, Marie Fierro at Manuch Aspen. The purpose of the inspection is to follow up on incident reports dated 04/12/26. LPA discussed with Facility Manager and Administrator, Levon Manucharyan details of the incident reported 04/12/26; LPA discussed their safety plan for all the youth in placement. The smoke detector located in the staff office will complete maintenance service on 4/15/26. No deficiencies were cited on this date. An exit interview was conducted and a copy of the report was provided to the facility.

      On April 14, Licensing Program Analyst (LPA) Miriam Flores met with Facility Manager, Marie Fierro and Administrator, Levon Manucharyan at Manuch Aspen. The purpose of the inspection is to follow up on incident reports dated 02/09/26, 3/18/26. and 4/4/26. LPA discussed with Facility Manager and Administrator details of the incident reported dated 2/09/26, confidential interviews indicated that direct care staff contacted law enforcement as soon as there was an AWOL with C1. Incident report dated 3/19/26, indicated that the facility updated the Needs and Services on 4/03/26 for C2. Incident reported dated 4/04/26, indicates that the incident occurred outside the facility while, C1, C2 and C3 were on AWOL status. Copies of incident reports were provided. No deficiencies were cited on this date. An exit interview was conducted and a copy of the report was provided to the facility.

      On November 5, Licensing Program Analyst (LPA) Miriam Flores met with Facility Manager, Marie Fierro at Manuch Aspen for the purpose of conducting a Case Management inspection. The purpose of the inspection is to follow up on an incident report dated 10/09/25. LPA discussed with Facility Manager details of the incident, which indicated the use of an electric metal detector upon youth in placement (C1) returning to the facility from an AWOL incident. The Department determined this to be a potential risk to health and safety of the children in care at this time, therefore California Code of Regulations are being cited on the attached LIC809D. The STRTP is being cited for the Interim Licensing Standards Section 87072(c)(10) Personal Rights. An exit interview was conducted, and a copy of the report and appeal rights were provided to the facility.

      On September 16, Licensing Program Analyst (LPA) Miriam Flores met with Facility Manager, Marie Fierro at Manuch Aspen for the purpose of conducting a Case Management. The purpose of the inspection is to discuss incidents dated 8/31/25, that indicated a medication error made by staff. LPA discussed with Facility Manager details of the incident. LPA highlighted the importance of administering medications appropriately. On 9/16/25, Facility Manager reported that S1 was terminated from Manuch Aspen STRTP. On 9/09/25, Manuch conducted an all staff training on Medication distribution. On 10/08/25, the facility will also be providing a second all staff training, conducted by Shouapa Thao on Psychotropic Medication Monitoring & Oversight. The Department determined this to be a potential risk to health and safety of the children in care at this time, therefore California Code of Regulations are being cited on the attached LIC809D. The Agency is being cited for the Interim Licensing Standards Section 87075(c) Health Related Services. An exit interview was conducted, and a copy of the report and appeal rights were provided to the facility.

      An unannounced Psychotropic Medication Inspection was conducted by Licensing Program Analyst (LPA) Briana Placencia on July 17, 2025 at 9:15am. This case management inspection is conducted pursuant to Health and Safety Code 1538.9(a)(2). During the inspection, LPA met with Levon Manucharyan, Administrator and inspected the facility inside and out. LPA reviewed 1 child's records and 2 staff personnel files on site at the facility. LPA reviewed 1 child's psychotropic medications and medication records. LPA interviewed 2 staff on site to discuss the facilities policies and procedures of operation. LPA interviewed 1 child currently psychotropic medication. LPA provided a copy of the licensing report to Administrator, Levon Manucharyan.. No deficiencies are being cited at this time. LPA conducted exit interview with Levon Manucharyan, Administrator.

      On June 10, 2025, Licensing Program Analyst (LPA) Miriam Flores conducted a case management visit at Manuch Aspen House STRTP and met with Facility Manager, Marie Fierro and Levon Manucharyan participated via phone. While conducting an investigation, it was determined that staff (see attached LIC811) was using inappropriate language towards youth in placement. The STRTP received a Technical Advisory Note and is being cited on LIC 87072(c)(1) Personal Rights, attached LIC 809D. An exit interview was conducted, and a copy of this report and appeal rights were left at the STRTP

      On June 10, Licensing Program Analyst (LPA) Miriam Flores conducted a complaint inspection at Manuch-Aspen House STRTP and met with Facility Manager, Marie Fierro and Levon Manucharyan participated via phone. The purpose of the inspection was to deliver the findings for the above complaint allegation. During the investigation, LPA conducted confidential interviews and reviewed supportive documentation which revealed that staff refused to transport youth in placement. Based on the information obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations are being cited on the attached LIC 9099D. An exit interview was conducted with facility staff. A copy of this report and appeal rights were left at the facility.

      On April 24, Licensing Program Analysts (LPAs) Miriam Flores and Sonia Chavez travelled to Manuch Inc Aspen House to conduct an Annual inspection. LPAs met with Facility Manager, Willie Williford and Administrator, Levon Manucharyan At 8:30AM, LPAs reviewed client's files and reviewed emergency intervention plan. There are two clients that require medications, facility has the ability to keep Medications locked and centrally stored. At 10:40AM, LPAs conducted an inspection of the physical plant and included the following checks. Facility grounds are clean and free of debris. Adequate indoor and outdoor activity space and materials were observed. Food supply, adequate bedding, storage space, activity space, hot water that measured 120 degrees F. Facility is maintained at 73 degrees. At 11:00AM, LPAs completed interviews with staff and made attempts to complete interviews with youth in placement. At 12:00PM, LPAs reviewed staff files. Youth in placement were present at the facility. Mental Health Program Approval expires on September 17, 2025. CARF accreditation expires on August 31,2025. Violations were cited on under ILS 87065.1(d)(6)(A) and CCR 80019(e). This facility was assessed for a civil penalty. See the attached LIC809D and LIC421BG. At 1:30PM, the exit interview was conducted with the Administrator and a copy of this report and appeal rights were provided.

      On July 30, Licensing Program Analysts (LPAs) Miriam Flores and Briana Placensia conducted a case management inspection at Manuch Aspen and met with Assistant Manager, Irvin Hermosillo. The purpose of the inspection was to discuss concerns with an incident report sent to Community Care Licensing (CCL) dated 07/24/2024 (0000850-FN), regarding a foster child #1 (see 811- Ref #1) reporting that AC was not cooling the house down fast enough. At arrival, temperature measured 72 degrees F. Foster child#1, declined to interviewed by LPAs. LPAs determined this to not be an immediate risk to health and safety at this time. No deficiencies are being cited. LPAs conducted exit interview with Assistant Manager, Irvin Hermosillo.
