On October 26, 2022, at 10:30 am, Licensing Program Analyst (LPA) Charmaine Linley arrived unannounced to the facility and met with A.B. Jessie Polinsky Children's Center Alicia Rogers, Protective Services Program Manager to discuss the investigative finding for the allegation noted above. LPA conducted an inspection of the facility on 09/02/2022 at 10:00 am. No deficiencies were observed. LPA interviewed one of one client (C1) and five out of five staff (S1-5). LPA reviewed the following documents: Daily Staffing Worksheet, dated 08/29/2022, Cottage log for 08/29/22, S1's employee evaluations 01/22/22 and 05/11/22, SIR's, and Video footage from the Gym dated 08/29/2022 from 4:00-5:00 pm. On 08/31/2022, Community Care Licensing (CCL) received an allegation that C1 was injured by staff due to excessive restraint. It was reported that S1 restrained C1, when C1 attempted to leave the facility and pushed and tussled with C1, resulting in bruises on C1's legs. Confidential interviews revealed that C1 spit water in S3's face and attempted to walk out of the Gym. Confidential interviews and video recordings reviewed revealed that S1 ran to C1 and pushed C1 back to the Gym wall. Video footage observed indicated that S1 pushed C1 against their shoulder back to the gym wall and S1 used their foot to knock C1's feet out from underneath them, causing C1 to fall on their buttocks. On September 28, 2022, at 5:00 pm, Licensing Program Analyst (LPA) Charmaine Linley arrived unannounced to the facility and met with Alicia Rogers, Protective Services Program Managerof A.B. Jessie Polinsky Children's Center to discuss the investigative finding for the allegation noted above. LPA conducted an inspection of the facility on September 1, 2022, at 9:40 am. No deficiencies were observed. During the investigation, LPA interviewed one of one client (C1) two of two staff (S1, S3), one of one County Social Worker (CSW1) and was unable to interview Maxim Healthcare employee (M1), despite multiple attempts to contact them. LPA reviewed the following documents: Daily staffing worksheets, contact information for M1, Nursing notes, and Cottage Log. On September 1, 2022, Community Care Licensing (CCL) received an allegation that staff handled C1 in a rough manner. It was reported that the minor was seen running out of a hallway and a Maxim staff grabbed the minor's clothing to stop them from running away, which caused the minor to fall on the ground. It was reported that the Maxim staff continued to hold onto the minor's clothing and redirected minor to the hallway. Confidential interviews revealed C1 had a history of running away from the facility. Confidential interviews reported C1 had run into traffic and shown unsafe behaviors when they run away from the facility.
