On 04/01/2026, Child A was restrained by Staff person A in response to control child's behavior. The child was placed in a standing upper torso by the staff person. Staff A had a physical dated and had no updated statement at the time of on-site review.

On 07/21/2023 child A was not treated with fairness, dignity and respect by staff member A. Staff A was observed intentionally stepping on the childs foot during their interaction. The staff kept foot on the childs foot for about 15 seconds. The child was observed pulling foot continuously with both hands until was able to free foot from under the staff.

During a CPS investigation, Staff Member A admitted that on 08/08/2023 after restraining Child A and returning with Child A to North Hall, Staff Member A removed all the child A's belongings from the child A's room as a punishment for the client. These belongings were not returned to Child A until 08/11/2023, when Staff Member B made sure the items were returned to Child A. The seizure of these items were outside of facility policy according to Staff Member B.
