      On 04/14/26 at 10:15am, Licensing Program Analyst (LPA) Simran Nayyar made an unannounced visit to the facility to conduct the annual inspection. LPA met Lauren Zeits, Clinician & Monico Jacobo, Program Assistant who assisted with the visit. LPA was informed that Program Director, David Jones was also on the way to the facility. There was five (5) staff and two (2) clients present in the facility. LPA toured the entire home, inside and out. The home is set up as follows: This is a one story home with two bedrooms, two bathrooms (one client bathroom and one staff bathroom), a living room, kitchen, garage (side tool/supply room not used by facility), and a basement being used for doing laundry. Bedroom # 1 - Occupied by one client (single full size bed, dresser, desk, and ceiling lighting). Bedroom # 2 - Occupied by one client (single full size bed, dresser, desk, and ceiling lighting). LPA observed the faucet in the client's shared bathroom delivered hot water at a temperature of 135. F, and the kitchen faucet temperature measured 132. F. LPA is issuing one Type B Violation under CCR 80088(e)(1).The facility has also been made aware of this issue and it is advised to put up warning signs advising clients of hot water temperatures. LPA found the facility to be clean and in good repair. All kitchen, food preparation, utensils, and storage areas were kept clean and free of litter. LPA observed food supply to be an ample amount.       An unannounced Psychotropic Medication Inspection was conducted by Licensing Program Analyst (LPA) Hanako Primer on February 3rd, 2026 at 10:50AM. This case management inspection is conducted pursuant to Health and Safety Code Section 1538.9 (a)(2). During the inspection, LPA met with Adeya Byrd, Administrator and Catherine Lau, Registered Nurse and toured the facility inside and out. Two clients are currently at the facility. One client left for an outing during the inspection. LPA reviewed one child's records and five staff personnel files on site at the facility. LPA reviewed one of child's psychotropic medications and medication records (PMAR). Based on record review, LPA found PMAR included multiple medications on one form and did not list all required components. LPA issued one technical violation under 87070(b)(14) Children's Records. See attached LIC9102 form. LPA interviewed two of staff on site to discuss the facilities policies and procedures of operation. LPA was unable to interview client due to their leaving for an outing during the inspection. LPA provided a copy of this licensing report to Catherine Lau, Registered Nurse.
