On August 7, 2025, the client's severe self-harm required transport to the emergency room for treatment and necessitated multiple sutures. Three days later, the client was not adequately supervised and self-harmed again by removing multiple sutures from the prior injury. This incident resulted in a second transport to the emergency room for additional treatment.

During the investigation inspection, the evidence substantiated that on September 17, 2025, a client with a history of significant self-injurious behavior engaged in a serious self-harm incident that resulted in a transport to the emergency room which necessitated over 30 sutures. A second client engaged in self-harm with the same contraband which also resulted in transport to the emergency room and sutures.

      The provider was out of compliance with R501-1-8(1)(a) for failing to maintain clean mattresses and ensure the environment is free from health hazards. During the inspection, the evidence substantiated mattresses with visible urine stains and what appeared to be vomit residue. Maintaining stained or contaminated bedding compromises facility sanitation and exposes clients to potential biohazards and unhealthy sleeping conditions. This is a repeat rule noncompliance as noted on February 25, 2024, December 5, 2024, April 10, 2025, and July 28, 2025.Technical Assistance was given on November 25, 2025 and July 28, 2025.
