These reports come from two Wyoming agencies. The Department of Family Services certifies residential treatment centers, group homes, crisis centers and residential schools and posts notices of non-compliance and facility visits: a notice records an allegation, whether the evidence supports it, and the rules broken. The Department of Health posts federal surveys of psychiatric residential treatment facilities in its survey search.

The state’s documents are scans. Notices are typed and their text was read by software, which makes reading errors; facility visits are handwritten forms and are shown as documents only, so a visit listed here may or may not have found violations. The state removes a provider’s documents when it leaves the list; the copies here stay.
