IRFU: 2957-FN On July 30th, 2026 at 1:30PM Licensing Program Analyst (LPA) Hanako Primer conducted a case management - incident inspection at the above licensed facility to follow up on an incident report received on April 28th, 2026. An initial inspection was made on May 5th, 2026. LPA met with Micki Hankes, Administrator, and explained the purpose of the visit. At the time of the inspection, there were five clients at the facility. Confidential interviews and record review could not confirm or dispute the allegation of a former staff member's inappropriate conduct. The staff member is no longer employed at the facility. Based on LPA's interviews, observations, and record review, although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove a violation did or did not occur. No deficiencies were issued at this time. An exit interview was conducted, and a copy of this report was left with Micki Hankes, Administrator, whose signature on this form confirms receipt.
