On 06/18/26 at 09: AM Licensing Program Analyst (LPA) Wendely Moodie-Naranjo made an unannounced visit to the above - listed facility for the purpose of conducting a case management incident inspection to investigate an incident report received on 06/05/26 by the Department concerning two clients (C1 and C2) engaging in sexual interaction. Per the SIR (Incident Report) On 06/03/26 client C2 snuck into C1's bedroom. Per SIR staff was being distracted by other client in the hallway to help C2 get into the room LPA met with (Theresa Scott, Facility Administror (FA), and Diana Dawson, Associate Director Youth Program (ADYP) who granted access to the facility. LPA requested LIC 500 and Client Roster as well as the camera waiver, a copy of the video footage at the time of the incident, Room Check logs from the day of the incident. Licensing staff conducted interviews with FA, ADYP and C1, observed footage of when C2 went into C1's room, [See Confidential Names form (LIC811), dated 06/18/26, for names]. Based on interviews and the review of the incident reports and video footage, the following deficiency was cited at the time of this inspection.
