Deficiency cited at inspection
On September 10, 2026, Licensing Program Analyst (LPA) Danny Macias arrived at Quality Family Services - Cambridge House (STRTP) to conduct an unannounced inspection and met with Head of Service (S1) Carole Whitty. The purpose of the inspection was to address an incident in which staff spoke inappropriately to a minor in care. LPA conducted staff interviews on this date. S1 states that the staff person has been placed on administrative leave pending investigation by the STRTP. S1 states that the staff admitted to STRTP management and law enforcement that staff did speak inappropriately to a minor in care. Therefore, the facility is being cited for violation of Interim Licensing Standards (ILS) Version 5, Article 06, Continuing Requirements, Section 87072(c)(16) Personal Rights on the attached LIC809-D. In addition, the facility is being issued a civil penalty for a repeat violation. A review of the Licensing Information System on this date indicates that all facility staff are associated to the STRTP and had DOJ, FBI, and child abuse index check clearances. An exit interview was conducted and a copy of this report and appeal rights was left with Carole Whitty.
