      An unannounced Psychotropic Medication Inspection was conducted by Licensing Program Analyst (LPA) Briana Placencia on May 5, 2025 at 9: AM. This case management inspection is conducted pursuant to Health and Safety Code 1538.9(a)(2). During the inspection, LPA met with Juan Quevedo, Administrator and inspected the facility inside and out. LPA reviewed 1 child record and 2 staff personnel files on site at the facility. LPA reviewed 1 child psychotropic medications and medication records. LPA interviewed 2 staff on site to discuss the facilities policies and procedures and plan of operation. LPA was unable to conduct any interviews as the child currently on psychotropic medication is at school, they will be interviewed at a later time. LPA provided a copy of this licensing report to Juan Quevedo, Administrator. No deficiencies are being cited at this time. LPA conducted exit interview with Juan Quevedo, Administrator

      An unannounced Psychotropic Medication Inspection was conducted by Licensing Program Analyst (LPA) Briana Placencia on May 5, 2025 at 9: AM. This case management inspection is conducted pursuant to Health and Safety Code 1538.9(a)(2). During the inspection, LPA met with Juan Quevedo, Administrator and inspected the facility inside and out. LPA reviewed 1 child record and 2 staff personnel files on site at the facility. LPA reviewed 1 child psychotropic medications and medication records. LPA interviewed 2 staff on site to discuss the facilities policies and procedures and plan of operation.       On January 23, 2025, Licensing Program Analysts (LPAs) Briana Placencia, Melissa Renovato and Olivia Medrano conducted an unannounced random inspection at Quality Clinton STRTP located in Fresno CA. LPAs met with Facility Manager Juan Quevedo. A physical plant inspection was completed and included the following checks. Adequate indoor and outdoor activity space and materials observed. The facility is maintained at 72 degrees. All facility smoke detectors are in appropriate working order, fire extinguisher is properly charged and serviced in March 19, 2024. Facility maintains an adequate supply of perishable and non-perishable foods. All required forms are posted. Medications are locked and centrally stored. Individual beds were observed. STRTP was approved on December 1, 2018. The facility is accredited through CARF with accreditation through May 31, 2025, and they are approved through Fresno County Department of Behavioral Health through June 1, 2026. Per Administrator there are no weapons/firearms in the facility. There are no bodies of water and no pets at the facility. LPAs reviewed 2 client files, and 0 staff files. LPAs interviewed 2 staff and no clients due to the clients being at school. LPA will follow up with staff files as well as interviewing the clients at school.       On June 18, 2021, Licensing Program Analyst (LPA) conducted an unannounced Case Management inspection to discuss an incident that occurred on 02/20/2021. LPA was met by facility representative Laura Romero. On 2/20/2021 a client in care was acting out in the facility office. The client became aggressive and two staff attempted to restrain the client. The LPA determined that when the two staff went to take down the client, the failed to use proper restraint protocol. Based on the above, the facility is being cited on the attached LIC 9099D. This report along with appeals rights are being provided to the facility.
