Deficiency cited at inspection

On October 17, 2024, Licensing Program Analysts (LPAs) Briana Placencia and Danny Macias conducted an unannounced required two year inspection at Quality Gettysburg STRTP located in Fresno CA. LPAs met with Facility Manager Joyce Wynn. A physical plant inspection was completed and included the following checks. Adequate indoor and outdoor activity space and materials observed. The facility is maintained at 73 degrees. All facility smoke detectors are in appropriate working order, fire extinguisher is properly charged and serviced in March 19, 2024. Facility maintains an adequate supply of perishable and non-perishable foods. All required forms are posted. Medications are locked and centrally stored. Individual beds were observed. STRTP was approved on December 1, 2018. The facility is accredited through CARF with accreditation through May 31, 2025, and they are approved through Fresno County Department of Behavioral Health through May 28, 2025. No firearms or weapons are observed in the facility. LPA reviewed 4 client files, and 5 staff files. LPA interviewed 1 staff and 2 clients refused to be interviewed. LPA discussed TB tests of clients, medication logs, needs and services plan with updates. Deficiencies are being cited on today's visit, that include two Type B violations. A copy of this report was left at the facility at the conclusion of today's inspection.

On September 17, 2024, Licensing Program Analysts (LPAs) Briana Placencia and Danny Macias conducted a Case Management inspection at Quality STRTP Gettysburg and met with Joyce Wynn. The purpose of this inspection was to discuss concerns with a late incident report that was sent into the Department on 8/30/2024, the incident in question happened on 07/17/2024. In speaking with Program Director he stated that it could have been an oversight and they are working with current staff to get the reports to PD for review in a timely manner. PD stated that there was a recent meeting held on 9/3/2024 to discuss these concerns. PD will continue to work/train staff to submit the documentation in a timely manner. The facility is being cited a Technical Assistance(TA). A copy of this report and TA was left at the facility.

LPA met with Facility Manager Austriana McCarty. A physical plant inspection was completed and included the following checks. Adequate indoor and outdoor activity space and materials observed. The facility is maintained at 73 degrees. All facility smoke detectors are in appropriate working order, fire extinguisher is properly charged and serviced in March 2023. Facility maintains an adequate supply of perishable and non-perishable foods. All required forms are posted. Medications are locked and centrally stored. Individual beds were observed. STRTP was approved on December 1, 2018. The facility is accredited through CARF with accreditation through May 31, 2025, and they are approved through Fresno County Department of Behavioral Health through May 28, 2024. No firearms or weapons are observed in the facility. LPA reviewed 3 client files, and 5 staff files. LPA interviewed 1 staff and 1 client. LPA discussed the medication logs, all required forms, intake process, needs and services plan with updates, STRTP Interagency Placement recommendation, disaster preparedness plan and emergency intervention plan. Deficiencies are being cited on today's visit, that include one technical violation. A copy of this report was left at the facility at the conclusion of today's inspection.
